Dear Parents/Guardians,
Welcome to the Academy of Our Lady of Mount Carmel School.
Please be aware that your child’s enrollment is not complete without a current physical examination (no more than 365 days prior to school entry) and up to date immunization records.
As per New Jersey state law, Flu vaccine is required for all pre-K students and is highly recommended for K through 8th grades. The Flu vaccine must be given between August and December 31st of the current year.
If your child requires medication of any kind during school hours, the medication must be sent to school in the original container and must be clearly labeled with the child’s name and instructions for administration. Please note, medications including over the counter and cough drops, must be accompanied by WRITTEN AUTHORIZATION from your child’s licensed healthcare provider (Doctor or Nurse Practitioner) and also signed by you, the parent/guardian. Medications may only be administered by the school nurse or a parent/guardian.
Please keep your child home from school if any of the following occur:
vomiting/diarrhea within 24 hours of the start of the school day
temperature >100 within 24 hours of the start of the school day
a positive diagnosis of pink eye or any bacterial infection (may return to school after 24 hours of treatment with appropriate medication).
Itchy head (Lice) may return 24 hours after treatment application with the treatment box top and are not (egg) free.
If your child is evaluated/treated in the emergency department for any reason, please contact the school nurse to be sure appropriate follow-up measures can be taken.
